What is MaineCare health Home program?
Health Homes (HH) includes MaineCare members who have two chronic conditions or one chronic condition and are at risk for another. Health Homes are a partnership between an enhanced Health Home primary care practice and one of ten Community Care Teams (CCTs) around the state.
What is Cub Care in Maine?
Maine’s Cub Care program is established to provide health coverage for low-income children who are ineligible for benefits under the Medicaid program and who meet the eligibility requirements. The Cub Care Programs provides Free or Low-Cost Health Insurance for Families with Children and Pregnant Women.
